Meeting Registration

Your registration is not final until you COMPLETE these 3 steps:

  1. Submit your online registration by completing the form below
  2. Email a copy of your valid license (Registered Nurse, Physician's Assistant, Registered Dietitian, Pharmacist, Physician, or Osteopath)
  3. Complete the attestation form verifying clinical practice hours using diabetes technology (hours must consist of using blood glucose monitoring, continuous glucose monitoring, insulin pump therapy, or software for insulin dose adjustments)

MEETING REGISTRATION RATES

Rates include attendance at the Clinical Diabetes Technology Meeting, April 29-30, 2016, and registration for the Certified Diabetes Technology Clinician (CDTC) certifying examination at the end of the meeting on April 30, 2016:

*All attendees will receive a 1-year subscription to Journal of Diabetes Science and Technology (an $800 value)

REGISTRATION POLICIES

The meeting is limited to 180 attendees and is expected to sell out.

A waiting list will be created after 180 registrants.

Registration is processed on a first-come, first-served basis.

On-site registration (if space permits) will incur an additional $100 fee.


REGISTRATION REFUND POLICIES

All refund requests must be made by contacting info@diabetestechnology.org

Refund requests postmarked/received before March 29, 2016 will receive a full refund, LESS a 25% processing fee.

Refund requests received after March 29, 2016 will not be honored.

Registration modifications will be processed at the prevailing rate at the time of modification.
